Payment Options

We accept all major credit and debit cards (Mastercard, Visa, AMEX, and Discover).  Flexible Spending Account (FSA) cards and Health Savings Account (HSA) cards are welcome!  You also have an option to pay in advance with a credit card or with your PayPal, or Square account for your visits when you schedule any available appointment online, although it is not required to pay up front to book your appointment.  We do not charge for no-shows, cancellations, late rescheduled appointments, etc.

Insurance

We accept most insurance plans.  Below are the plans for which we are a participating, in-network provider:

  • Aetna
  • American Specialty Health (ASH)    
  • Anthem Blue Cross
  • Blue Shield of CA
  • Cigna
  • HealthNet
  • Kaiser Permanente (with Chiropractic coverage through ASH)
  • Medicare
  • United Healthcare / UMR

We can verify your benefits prior to coming in for your first visit.  Click here to be taken to our New Patient Intake Page.  There, you can complete your basic information to include your insurance carrier, insurance ID and group number (if applicable).  Once you submit, we securely receive the information to verify your eligibility.  Please be sure to bring your insurance ID card and valid photo ID to our office on your first visit. 

Medical Payments and Liens

Medical Payments or "Med-Pay" can immediately cover the costs of medical expenses associated with injuries sustained in a car accident.  Med-Pay is part of your own auto insurance policy, separate from "comprehensive coverage." 

If you would like our office to bill your Med-Pay for your accident-related injuries please provide us with the name of your auto insurance company, adjuster's name and contact information, and injury claim number (this is different from your property damage claim number) when you come in for your first post-accident visit.

If you do not have Med-Pay available, payment is due at time of service.  Upon request and with your written authorization, we can provide to you your complete records and billing statements so you may seek reimbursement from the responsible party/insurance company.  We do not bill the other party, even if they have accepted fault.
